I, the undersigned, acknowledge that during the Hearing Aid Academy Bootcamp, I will participate in practical exercises, including ear mold impression activities. I understand that these activities involve inserting impression material into the ear under professional supervision.

I acknowledge that while the instructors provide guidance and oversight to ensure safe practices, there remains a small inherent risk of discomfort, injury, or other complications. By participating, I voluntarily assume all such risks.

I hereby release and hold harmless the Hearing Aid Academy, its instructors, and affiliates from any liability for any injury, damage, or harm that may occur as a result of these activities. I confirm that I will follow all instructions and safety protocols provided.
