Qualifications For Licensure Hearing Aid Dispensers

  • Must submit to fingerprinting.

  • Be at least 18 years of age.

  • Be a high school graduate (or equivalency).

  • Pass California hearing aid written and practical exams.

THREE PATHWAYS TO LICENSURE

1. Applicant/Written Exam- No License

This pathway to licensure requires you to pass both the California written and practical exams to obtain your full license. You are not allowed to work without a license.

Fees

  • Application fee – ($75)
  • Written exam fee – ($225)

Fingerprints – DOJ and FBI clearances must be received prior toissuance of a license.

  • California applicants are required to use Live Scan for fingerprinting;please submit a copy of the completed form to the Board. Fees are paiddirectly to the Live Scan operator.
  • Out-of-State applicants are required to submit two fingerprint cards(FD-258) and a check or money order to the Board for $49.00 (DOJ andFBI processing fee).

For out-of-state applicants, one (1) check or money order in theamount of $349 ($300 application and written exam fee and $49fingerprint card processing fees) may be submitted. Please makecheck or money order payable to SLPAHADB.

High School Diploma

  • Please submit a copy of your high school diploma, GED, or highereducation diploma with your application.

Government Issued ID verifying a minimum of 18 years of age

  • Please submit a copy of a Driver’s License, Passport, ID card, etc.

2. Trainee with Supervision – Temporary License

This pathway to licensure requires you to find a hearing aid dispenser who has been licensed at least three years in California to become your supervisor. The trainee temporary license is issued for six months and can be renewed two times. As a trainee, you must take the written exam within the first 10 months of issuance.

  • Application fee ($175)
  • Written exam fee ($225)

Fingerprints – DOJ and FBI clearances must be received prior to issuance of a license.

  • California applicants are required to use Live Scan for fingerprinting; please submit a copy of the completed form to the Board. Fees are paid directly to the Live Scan operator.
  • Out-of-State applicants are required to submit two fingerprint cards (FD-258) and a check or money order to the Board for $49.00 (DOJ and FBI processing fee).

For out-of-state applicants, one (1) check or money order in theamount of $449 ($400 application and written exam fee and $49fingerprint card processing fees) may be submitted. Please makecheck or money order payable to SLPAHADB.

3. Licensed in Another State – Temporary License

You must possess an active and current hearing aid dispensing license from another state. This temporary license is issued for 12 months and allows you to work as a hearing aid dispenser. It is not renewable. During these 12 months you must take and pass both the California written and practical exams. If you fail either exam, you must immediately surrender your temporary license.

State Licensure Verification

  • Submitted from each state where you were/are licensed.
  • Must be original letter from licensing entity.

Employment Verification

Verification on employer’s letterhead that you have been engaged in the fitting andsale of hearing aids for the two years immediately prior to applying for a CaliforniaLicense.

If self-employed, please confirm that you have engaged in the fitting and sale ofhearing aids for the two years immediately prior to application. Must be on businessletterhead.
